Please read these Terms and Conditions carefully before using this portal.

Polishmedic S.A. is a free service to prospective patients.  Polishmedic S.A . is not a healthcare provider, medical advisor or a medical referral service.

1. Polishmedic S.A., its owners, employees and representatives, are not liable for any damages that may arise to a prospective patient either before, during or after a medical procedure(s) is/are performed.  Polishmedic S.A., its owners, employees and representatives, are not liable for any future problems that may arise from any medical procedure(s) that was performed in Poland.  Polishmedic S.A. , its owners, employees and representatives, are not liable for any problems that may arise during a patient's stay in Poland. If a patient deems their medical procedure(s) unsatisfactory or believes that a doctor or hospital/clinic was negligent, they have legal recourse in the Polish legal system.
2. The owner of the portal declares that, in order to use this portal, the acceptance of these regulations is essential. Should a user not consent to any of the provisions set out hereinafter whatsoever, they should immediately cease to using this portal.
3. The information included on the website may not be taken by the user as being an offer in the sense of the Civil Code of 23rd April 1964.
4. The owner of the portal declares itself to be the owner of the copyrights to, or licences to use, any and all of the contents included on the portal which constitute a work in the sense of the Copyright and Neighbouring Rights Act of 4th February 1994. The owner of the portal gives its consent to the user's non-commercial, private use of these materials. Any and all other use of the said materials is prohibited and will result in the owner of the portal's vindication of its rights under the applicable regulations.
5. The owner of the portal declares that the contents published on the website are strictly and solely intended to serve information purposes and should not be taken as being medical advice. Their application on the part of the user for medical purposes in any way whatsoever may not give rise to liabilities in any form whatsoever on the part of the owner of the portal.
6. The owner of the portal shall make every effort to ensure that the contents included on the portal are up to date and accord with the current status; however, it cannot guarantee this. The application, in any way whatsoever, on the part of the user of out-of-date information or information which does not accord with the current status may not give rise to liabilities in any form whatsoever on the part of the owner of the portal.
7. The owner of the portal does not guarantee the veracity of the information included thereon, this being the property of the third parties to whom the portal provides links. The owner of the portal is neither responsible nor accountable for the contents published on the portals of third parties.
8. Any and all information essential to the performance of the intermediary services provided by the owner of the portal between the patient and the selected clinic will be furnished by the said owner by means of e-mail.
9. It is essential that the user forward their personal details to the said owner, using the form published on the portal; the aforementioned data include: Given name, Surname, Telephone number, e-mail address
   The owner of the portal declares that the data thus provided may be forwarded to a third party strictly and solely for the purpose of performing the services set out under Item 8 herein. In sending their personal details, the user gives their consent to the processing of the said details by accepting the following clause: "I hereby consent to the processing of such of my personal data as are essential to the owner of the portal's undertaking work prior to the concluding of an agreement on the provision of medical services, in accordance with the Act on the Protection of Personal Data of 29th August 1997 (Dz. U. [Journal of Laws] No. 133, item 883). I further consent to the owner of the portal's forwarding to third parties such of my personal data as are essential to the owner of the portal's undertaking work prior to the concluding of an agreement on the provision of medical services, in accordance with the Act on the Protection of Personal Data of 29th August 1997 (Dz. U. [Journal of Laws] No. 133, item 883)." The owner of the portal gives notice that, in accordance with the provisions of the Act on the Protection of Personal Data of 29th August 1997, personal details are made available to the said owner voluntarily, while the user has the right of access to the substance of their data and to its emendment. With a view to exercising that right, the user should contact the owner of the portal by means of e-mail.
10. In sending a query to the owner of the portal, the user gives their consent to the forwarding of commercial information in the sense of the Act on the Provision of Services by Electronic Means of 18th July 2002, by accepting the following clause: "I hereby give my consent to the forwarding of commercial information on the part of the Polishmedic S.A.  for Information on Clients Interested in Medical Services, having its registered office at Żurawia st. Warszawa 00-515, in accordance with the Act on the Provision of Services by Electronic Means of 18th July 2002 (Dz. U. [Journal of Laws] of 2002, No. 144, item 1204, with further amendments)."
11. The user will be notified by means of e-mail as to the provision of any and all other information essential to the performance of the services, such as, for example, the results of tests. The owner of the portal declares that this data may also be forwarded to a third party strictly and solely for the purpose of performing the said services.
